• Limit access to PHI to only those with a need-to-know basis.
• Encrypt PHI at rest and in transit.
• Train personnel on HIPAA and PHI responsibilities.
• Securely dispose of paper and electronic PHI.
• Notify affected parties of reportable incidents under HIPAA Breach Notification Rule.
